We are seeking to hire a Registered Nurse to be part of our Care Coordination/Case Management Team. St. Louis County offers competitive pay and a robust benefits package.
Duties of this role include:
- Coordinating care and services to clients age 65+ that are enrolled with Blue Cross and Blue Shield managed care plans.
- Completing comprehensive health risk assessments at least annually for health and social needs.
- Developing, reviewing, and maintaining individualized, comprehensive care plan for clients.
- Serving as the client's advocate.
This full-time position could be based in Duluth, Hibbing, or Virginia, MN based on the preference of the selected candidate.
The salary range reflected above is the normal hiring range for 2026. The full salary range, including longevity pay, is $31.21 - $44.23 per hour.
These positions are part of the Civil Service Basic Unit Employees Collective Bargaining Agreement represented by Council 5 of AFSCME.

An employee in this class is responsible for reviewing client needs as required according to federal and state program eligibility criteria. Duties may include providing comprehensive assessments, educational counseling, and providing case management and care coordination depending on the program area. The work is performed under the direction of assigned supervisory staff.

OCCASIONAL: Lift/carry and push/pull up to 10 pounds; grasp; move about; climb stairs; stand; far vision; works alone; works with/around others; customer/public contact; clients with behavioral challenges; outside; drive/traffic; exposure to slippery surfaces; and exposure to weather when traveling between office and other locations.
RARE: extended work day.
- Graduation from an accredited college or university with an Associate’s degree in nursing.
- Possession of a current Minnesota Registered Nurse license.
- Possession of a valid driver’s license.
HOME AND COMMUNITY – BASED & PUBLIC HEALTH MANAGED CARE PROGRAMS:
- Two (2) years full-time paid verifiable experience in a home or community-based setting.
- Possession of MnCHOICES Certified Assessors certification within two (2) months of employment. Failure to obtain the MnCHOICES Certified Assessor certification within two (2) months will result in termination from the position.
Experience & Training Rating - 100% of the scoring process:
- Points will be awarded for job-related experience and education exceeding the minimum qualifications. Only experience occurring during the ten most recent years will be used in determining this assessment. Experience beyond ten years may be included on the application for purposes of meeting minimum qualifications or informing the hiring authority of your complete employment history.
SCHEDULE:

- Week of August 31, 2026 - Experience & training scoring to be completed
- Week of September 14, 2026 - Interviews to be conducted
