Position: CLINICAL LIAISON - HOSPICE ELECTION PROGRAM

CLINICAL LIAISON – Hospice Election Program

Location:

Aspirus St. Luke's Hospital, Duluth, MN.

Salary Range: $33.75 - $48.24 hourly. Schedule/

Hours:

Day only, 8 hours. Weekend Requirement:
Generally no weekends. FTE:
Full time, 1.0 FTE.

Responsibilities

The Clinical Liaison collaborates with the Hospice Medical Director, Palliative Care team, Home Health or Hospice leadership team, and inpatient providers to determine home health or hospice eligibility for patients within the Aspirus system hospitals. Works closely with the care coordination team to ensure an appropriate and timely discharge for Hospice and Home Health patients. The primary focus of this position is to increase face‑to‑face conversations with patients and families regarding Home Health and Hospice Services and the benefit to their quality of life.

This position requires an understanding of the importance of home health and hospice goals and the impact they have on system initiatives.

Minimum Knowledge, Skills, and Abilities Education
  • Associate degree in nursing required. Bachelor's degree in nursing preferred.
Experience
  • One year of hospice experience required. Two years of hospice experience preferred. Home health experience is beneficial.
Driver’s License Required
  • Possesses a valid Driver's License required.
  • Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required.
Other
  • Ability to meet, collaborate, and work efficiently with others.
  • Strong patient advocacy skills.
  • Ability to review clinical documentation for accuracy, thoroughness, and completeness with fine attention to detail.
  • Demonstrates ability to write professionally, work independently, and communicate with individuals and groups.
  • Demonstrates application of teaching/learning principles.
  • Demonstrates an understanding of the needs of various age groups in terms of their physical, motor, psychosocial, and intellectual/language development as it applies to volunteer employee care and services.
