Job Title: Admissions Coordinator

Department: Admissions/Marketing

Reports To: Administrator or Director of Admissions

The Admissions Coordinator is responsible for managing the resident admission process for a nursing home or skilled nursing facility. This role serves as the primary contact for prospective residents, families, hospitals, referral sources, and healthcare professionals to ensure timely, accurate, and efficient admissions while maintaining census goals.

• Coordinate all resident admissions from hospitals, clinics, assisted living facilities, and community referrals.
• Review referral information and determine admission eligibility based on facility capabilities, bed availability, and payer sources.
• Communicate with hospitals, case managers, discharge planners, physicians, residents, and families regarding placement options.
• Obtain and verify insurance coverage, Medicare, Medicaid, managed care, and private-pay information.
• Complete admission paperwork and ensure regulatory compliance.
• Conduct facility tours for prospective residents and families.
• Maintain accurate admission, referral, and inquiry tracking records.
• Collaborate with nursing, therapy, social services, dietary, and business office departments to facilitate smooth resident transitions.
• Promote positive relationships with referral sources and community partners.
• Participate in census development and occupancy initiatives.
• Assist with marketing and community outreach activities as assigned.
• Ensure confidentiality and compliance with HIPAA and other applicable regulations.

• Associate's degree in Nursing
• Experience in long-term care, skilled nursing, assisted living, hospital discharge planning, or healthcare admissions preferred.
• Knowledge of Medicare, Medicaid, managed care, and private-pay reimbursement.
• Strong communication, customer service, and organizational skills.
• Proficiency with electronic health records and Microsoft Office applications.
• Ability to work effectively with residents, families, and healthcare professionals.
