We are seeking an experienced Nurse Practitioner to provide high-quality, patient-centered primary care in Skilled Nursing Facilities, LTC, and ALFs.
Responsibilities- Perform assessments, exams, and chart reviews; develop and adjust treatment plans.
- Manage acute and chronic conditions, prescribe medications, and interpret diagnostics.
- Educate patients, families, and staff to promote health and prevention.
- Collaborate with team members to ensure seamless care and support quality improvement.
Minnesota NP license, Board Certification, 1+ year experience preferred, strong geriatric and primary care background, excellent communication skills.
