In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Willmar, MN

• Provide In‑Home Health Evaluations (~45 minutes) including medication and medical history review, physical evaluation, and additional diagnostic tests as ordered by the health plan.
• Use an iPad and intuitive clinical workflow for each evaluation; not required to prescribe medicine, order lab tests or alter current treatment.
• Build trusted relationships and connect patients to the right care for their unique needs.
• Conduct virtual visits as needed.
• Perform diagnostic and preventive services such as spirometry, diabetic eye exams (DEE), bone density screening, etc.

Primary workplace is the private residence of members; environments may contain pets, secondhand smoke, or allergens. Candidates must be able to work safely in such conditions.

• Active, unrestricted license(s) in coverage area(s).
• Completion of 2080 hours post‑graduate collaborative practice in an integrated clinical care setting (required).
• Board certification (required).
• Multi‑state licenses (preferred).

40 hours, part‑time.

$41.85 – $90.13 per hour. The actual offer will depend on experience, education, geography and other factors.
