Licensed Practical Nurse (LPN) – Home Care

HoursFull‑time | Monday – Friday | Weekend and holiday rotation. Dependent on staffing levels, work approximately one weekend every 4‑6 weeks and approximately 2 holidays per year.

The LPN will have a general territory within our service area where they will provide one on one in‑home care to the patients. The office is in Roseville, MN, and the primary territory for this position would be in the southeast section of the Twin Cities metro area.

- Current license as a Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
- 2 years’ experience as an LPN preferred.
- Ability to travel within geographic area serviced by the office. Valid driver’s license, reliable transportation, and proof of auto insurance required.
- CPR certification.
- Participate in ongoing interdisciplinary assessment of the patient.
- Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Provide services that are ordered by the physician as indicated in the plan of care.
- Recognize and report changes in the patient’s condition to the RN supervisor.
- Counsel and educate the patient and family in meeting healthcare related needs.
- Complete timely and accurate clinical notes including addressing patient’s progress.
