Various, in person in selected MDH District Office sites (Bemidji, Duluth, Fergus Falls, Marshall, Rochester, St. Cloud, or St. Paul) with telework and travel up to 50% of the time - Work Hours:
Monday‑Friday, 8:00 a.m. – 4:30 p.m.; early morning, evening, or weekend work may be required based on investigation workload - Salary: $37.82 – $55.93 /hour ($78,968 – $116,781 /annually)

- Investigate allegations of non‑compliance or maltreatment in facilities licensed or certified by MDH
- Evaluate provider compliance with state and federal rules and identify deficiencies
- Compile and document investigation findings, including statements of deficiencies and compliance status
- Conduct unannounced follow‑up site visits to assess corrective orders
- Attend legal proceedings and testify on behalf of MDH when required
- Licensed as a Registered Nurse (RN) in Minnesota, without restriction
- Two (2) years of professional experience in a health‑care setting, preferably:
- Registered Nurse (RN)
- Charge Nurse (minimum two years)
- RN Supervisor, Director of Nursing, or Health‑Care Administrator
- Educational experience may substitute for required professional experience:
- Doctorate: 24 months of experience
- Master’s: 18 months
- Bachelor’s: 12 months
- Associate: 6 months
- In-depth knowledge of applicable nursing standards, rules and regulations
- Ability to manage multiple investigations across different license types
- Strong written and verbal communication skills
- Excellent interpersonal and collaborative skills
- Proficient in computer use for documentation and report writing
- Successful completion of a background check and employment reference check
- Clinical or managerial experience in assisted living facilities or home care settings
- Experience conducting regulatory, licensing, or health‑care investigations
- Experience working with diverse populations, including considerations of religion and gender identity
- Multilingual or bilingual ability
