Home Care Registered Nurse, RN

Home Care Registered Nurse, RN—Part Time or Full Time Day Shift 8:00 AM – 4:30 PM with flexible schedule and occasional weekend/holiday requirements.

Part Time or Full Time weekday shift 8:00 AM – 4:30 PM, occasional weekend/holiday requirements.

Pay:
Targeted range $38.50 – $41.50 per hour, determined by experience, qualifications, certifications, licenses, and education.

• Makes visits to clients’ homes to provide nursing care based on needs and care plan. Visits may be scheduled or as needed.
• Assesses client condition and reports concerns to the client’s Ecumen RN Case Manager; collaborates to update the care plan.
• Administers medications and treatments per provider’s prescription, follows plan of care, completes charting as appropriate.
• Communicates with medical providers regarding client needs and/or changes in condition; seeks orders as required.
• Provides health‑care instructions to clients per assessment and care plan.
• Receives and responds to phone calls during assigned shifts.

• Graduate of an accredited Registered Nursing program.
• At least 1 year of nursing experience in a post‑acute care setting.
• Licensed as a Registered Nurse in the state of employment and maintain licensure.
• Valid driver’s license, proof of insurance, satisfactory motor vehicle record.
• Ability to communicate effectively, both verbally and in writing.
• Proficient with Microsoft Office Suite and OASIS documentation.
• Ability to follow all safety rules, regulations, policies, and procedures of the facility.

• Bachelor of Science degree in Nursing.
• One year nursing experience in Home Care.
• Public Health Nurse certificate.
