We are a growing, physician‑owned medical group expanding in Duluth, MN, seeking an experienced Nurse Practitioner (NP) to join our team in a Skilled Nursing Facility (SNF) setting. This role focuses on post‑acute and rehabilitative care, including orthopedic recovery, stroke management, and pain management, working alongside a collaborative lead provider.

• Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning
• Manage patients with orthopedic rehab needs, post‑stroke care, and chronic pain conditions
• Ensure accurate and timely documentation in EMR

• Active Nurse Practitioner license in Minnesota
• Active DEA license (or ability to obtain prior to start)
• Minimum 1 year of clinical experience as an NP (SNF, geriatrics, internal medicine, or rehab experience strongly preferred)
• Experience managing adult/geriatric patient populations
• Strong clinical decision‑making and time‑management skills
• Comfortable working independently in an SNF setting

• Skilled Nursing Facility (SNF) or post‑acute care experience
• Background in internal medicine, geriatrics, or physical medicine & rehabilitation (PM&R)
• Experience with orthopedic, stroke, or pain management patients

• Part‑time: 2‑3 days per week
• No nights, no weekends

• Must be able to lift up to 50 pounds

• Must wear the required PPE per company and/or facility requirements
• Must comply with facility vaccination requirements
