Job Overview

Signify Health, part of CVS Health, seeks a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

• Conduct a 45‑minute visit that includes medication and medical history review, a physical evaluation, and additional diagnostic tests as ordered by the person’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Do not prescribe medicine, order lab tests, or alter current treatment regimens.
• Bring a caring presence to every visit, contributing to a national network of purpose‑driven clinicians.
• Assist members in connecting to the right care for their unique needs.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry and diabetic eye exams.

As an in‑home clinician, the primary workplace is the private residence of health plan members. Candidates must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Active, unrestricted license(s) in the coverage area(s).
• Completion of 2,080 hours of post‑graduate collaborative practice hours as an NP or PA in an integrated clinical care setting (and filing the appropriate attestation with the state of Minnesota).
• Board certification (required).
• Multi‑state licenses (preferred).

Anticipated weekly hours: 29.5 hours. Time type:
Part time.

The typical pay range for this role is $41.85 - $90.13 per hour. The actual base salary will depend on experience, education, geography, and other relevant factors.
