Overview

Home Care Registered Nurse Case Manager, RN – Full Time (1.0 FTE). On-call rotation required; schedule typically Monday–Friday, 8:00 am–4:30 pm, with every 5th weekend rotation. Locations include Duluth, Knife River, Two Harbors, Carlton, Cloquet, Wресnshall, Esko, Saginaw and Hermantown. Responsibilities

Delivers skilled nursing services efficiently and effectively to meet client and family needs.

Performs initial, comprehensive, and ongoing assessments of clients and families to determine needs, including a complete physical assessment and history.

Collaborates with physicians, other health care professionals, clients, and families to develop a comprehensive, coordinated plan of care.

Develops a plan of care with objective goals for health and functional status; evaluates and updates the plan of care and informs physicians of changes.

Ensures physician orders are communicated accurately and thoroughly to the agency staff.

Initiates case conferences to discuss multidisciplinary team responsibilities, client progress, and plans for continued care and any new problems.

Promotes personal safety and a safe environment for clients and co-workers.

Minimum Qualifications

1 year of nursing experience in a post-acute care setting.
Licensed as a Registered Nurse in the state of practice and able to maintain licensure.
Effective verbal and written communication skills.
Proficiency with Microsoft Office Suite.
Ability to follow safety rules, policies, and procedures (including HIPAA, Universal Precautions, OSHA, rights and safety policies).

Licenses & Certifications

Registered Nurse (RN) – active license in the state of practice.

Preferred Qualifications

Bachelor’s degree in Nursing.
Previous experience working with seniors.
