As the Admissions Nurse, you are the clinical quarterback, guiding the start of care and ensuring every patient begins with a clear, safe, and effective plan. This is ideal for nurses who want to step into a more strategic, assessment-driven role without moving into management.

What You’ll Own
  • Complete Start of Care (SOC) admissions, delivering thorough in-home assessments and accurate OASIS documentation.
  • Develop and initiate individualized care plans, ensuring each patient is set up for safe and effective home health services.
  • Identify risks, gaps, and service needs early, coordinating the right disciplines and resources from the start.
  • Communicate clearly with providers, caregivers, and internal teams to ensure alignment and continuity of care.
  • Set the tone for quality, compliance, and patient experience from the very first visit.
What Sets You Apart
  • Active Minnesota RN license with strong clinical foundation.
  • Proven experience with home health admissions and OASIS documentation.
  • Ability to think critically, prioritize quickly, and make confident decisions independently.
  • Strong communication skills with a focus on patient education and care coordination.
  • Reliable transportation and comfort working in a field-based environment.
