In the heart of our mission, our RNs play a vital role in overseeing and delivering high-quality nursing care to our residents. Your duties will include:

• Provide Direct and Supervisory Nursing Care in accordance with Care Plans and Physician Orders
• Administer Medications and Treatments Safely and Accurately
• Perform Comprehensive Assessments and Update Care Plans
• Monitor Resident Conditions and Respond to Changes with Clinical Judgment
• Supervise, Guide, and Support LPNs and CNAs to Ensure Quality Care Delivery
• Maintain Accurate and Timely Documentation in Compliance with Regulations
• Communicate Effectively with Physicians, Families, and Interdisciplinary Team Members
• Ensure Compliance with All State, Federal, and Company Policies and Procedures

Qualifications:

• Current RN License in Good Standing
• Strong Clinical, Critical Thinking, and Assessment Skills
• Previous Skilled Nursing or Long-Term Care Experience Preferred
• Ability to Work Effectively Both as a Team and Independently
• Leadership Skills with a Supportive and Collaborative Approach

Competitive salary: $45.00 – $48.00/hr, commensurate with experience.

Location:

Just west of the Denver Metro Area, near Lakewood

Pay: $7,000 Sign-on bonus for Full-time RN;
Days, Evenings or Nights.

Benefits:
Mileage program for those 25 miles away or more, Shift Differential for NOC, Extra Shift bonus availability.

Key responsibilities:

• Provide and direct nursing care to patients in homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
• Coordinate total plan of care with other health care professionals.
• Document and care delivery that meets infection control standards.
• Provide supervisory visits and support teams.

Education, Experience, and Licensure Requirements:

• Associate’s or bachelor’s degree in nursing.
• Currently licensed/registered in applicable State; must maintain active RN license in good standing.
• One (1) year skilled nursing experience preferred.
• CPR certification upon hire or obtain during orientation; must remain current.

Annual total cash target pay: $82,995 – $124,492

Hourly rate: $39.90 – $59.85

Per visit point: $47.88 – $71.82

Primary Responsibilities:

• Clinical Competence – Initiate, develop, implement, and revise the plan of care.
• Documentation and Care Delivery – Provide high-quality clinical services within scope of practice.
• Quality – Ensure patients meet eligibility and medical necessity guidelines, submit accurate documentation.
• Teamwork – Assist in orientation, participate in surveys, adhere to agency utilization model.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.

Required Qualifications:

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Preferred Qualifications:

• Current CPR Certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.
