• Assess health problems and needs of persons served, develop and implement nursing care plans, and maintain medical records.
• Administer nursing care and advise persons served and staff on health maintenance and disease prevention.
• Provide case management.
• Review daily appointment needs, nursing calendar, and ensure coverage.
• Monitor staff activities, medication administration, meals, ADLs, and plan implementation.
• Review incident reports, appointment findings, and implement necessary changes.
• Approve medication and treatment orders, monitor health status, and address ongoing health issues.
• Attend IPPs, review MARs, and provide on-call services.
• Perform assessments such as fall risk, skin integrity, and choking.
• File healthcare documents, review medication storage, and manage medication orders.
• Conduct orientation and complete monthly nursing summaries.
• Develop and update medication support plans.

• Degree from an accredited nursing school or current RN license in good standing.
• Valid driver’s license.
• At least 18 years old with one year of nursing experience; supervisory experience preferred.
• Minimal travel required.
