Licensed Practical Nurse

Dunbar Center in Youngstown, OH is seeking a Licensed Practical Nurse to provide compassionate, resident-centered care in our skilled nursing and rehabilitation facility. The LPN will deliver hands-on nursing care, administer medications and treatments, monitor residents' conditions, and accurately document in the EHR. You will collaborate with an interdisciplinary team to support personalized rehab and long-term care plans, promote safety, dignity, and comfort, and help residents maintain their highest level of independence in a warm, homelike environment.

Responsibilities

• Provide direct bedside care, including ADL assistance and resident monitoring
• Administer medications and treatments per physician orders
• Monitor and document vital signs, changes in condition, and care provided in EHRCollaborate with RNs, therapists, and physicians on care plans
• Educate residents and families on care, treatment, and safety
• Follow infection control and safety protocols at all times
• Support rehab and restorative care goals to maximize resident function
• Participate in shift reports and communicate resident status to the care team
