Float Pool Licensed Practical Nurse

Dunbar Center is seeking a Float Pool Licensed Practical Nurse to provide high-quality resident-centered care across units in our skilled nursing and rehabilitation facility. The LPN will deliver bedside care, administer medications and treatments, monitor residents' conditions, document care accurately, and collaborate with RNs, therapists, and physicians. Responsibilities include supporting rehab plans, assisting with ADLs, responding to changes in condition, and educating residents and families.

This role requires flexibility to float between units, strong communication, and a commitment to compassion, dignity, and safety.

Responsibilities

• Provide bedside nursing care to residents across multiple units
• Administer medications and treatments per provider orders
• Monitor and document residents' conditions and responses to care
• Support rehabilitation and care plans in collaboration with the care team
• Assist residents with activities of daily living (ADLs)
• Respond promptly to changes in resident status and notify RN/physician
• Maintain accurate, timely clinical documentation and reports
• Communicate with residents and families regarding care and instructions
• Follow infection control, safety, and regulatory guidelines
• Float between units as needed to support staffing and resident needs
