DescriptionUnder general direction, the Program Therapist oversees mental health treatment for senior adults as ordered by the medical director;
Participates as part of a multidisciplinary team to complete case management, treatment planning, diagnostic assessments, and discharge planning for senior adults in a behavioral health setting.
RESPONSIBILITIES (ESSENTIAL FUNCTIONS):
- Conducts multiple group therapy sessions daily as well as individual and family therapy sessions as ordered/assigned at outpatient behavioral health (Inspirations) and inpatient (Horizons).
- Monitors and evaluates patient progress through completion of patient assessments for mental illness and cognitive changes;
Completes daily group therapy session notes, psychosocial assessments, initial treatment plans, monthly treatment plan reviews, and discharge summaries. - Collaborates with outside agencies, peers, and/or family as appropriate to coordinate patient care.
- Assists the Manager in development and maintenance of effective quality control and performance improvement processes;
Completes initial, monthly, and discharge audits of charts;
Tracks patient outcomes. - Collaborates with the medical director, program manager, and unit nurse as part of multidisciplinary treatment team to meet patient needs and coordinate care. Attends weekly team meetings.
- Recognizes patients at risk for harming self or others and takes action to promote safety;
Implements crisis intervention and de-escalation skills, as necessary. - Orients and trains new team members on departmental policies and procedures;
Maintains appropriate licensure and credentials;
Attends CPI and CPR semi-annually. - Safely transport patients from their residence to the IOP (and back).
- Utilize 4-point wheelchair restraints and seatbelts to safely secure all patients
- Assist with serving patient lunch and snack break.
- Maintain and assure all medical equipment is operational/reliable by maintaining written record/inventory of all equipment on a monthly audit log to verify the equipment is operational reliable, address special risks and respond to failures.
- Regular attendance and punctuality for scheduled shifts.
- Appropriately adapts assessment, treatment, and/or care methods to accommodate the physical, cultural, age-specific, and other developmental needs of each person served.
- Maintains professional and technical knowledge through continuing education opportunities including internal and external educational offerings.
- Utilization of assistive devices for lifting is mandatory.
- Per DRH policy, all required conditions of employment must be met and maintained including required vaccinations.
- This position requires respirator and fit testing. Required testing will be performed on all team members at risk for exposure to airborne infectious diseases, droplet producing procedures, or any caustic chemicals.
- Must adhere to, and promote, safety protocols at all times.
- Implement DRH Standards of Behavior and exhibit behaviors consistent with DRH core values.
- Performs other related duties as assigned.
Minimum Qualifications:
Communication skills including fluency in oral and written English. Basic computer skills including the ability to send/receive/email, navigate information technology associated with the position, and use Electronic Health Record information tools. Excellent written, verbal, and interpersonal communication skills;
Ability to remain flexible to quickly adapt to urgent situations. Ability to adapt procedures, processes, tools, equipment, and techniques to accomplish the requirements of the position. Ability to travel to various locations around the facility. This position requires advanced fingerprint background checks.
Education and/or
Experience:
A graduate degree is required. One year experience in facilitating group, family, and individual counseling sessions preferred. Previous experience in treating senior adults preferred. Ability to apply the skills of self-motivation and project management. Ability to read, interpret and apply policies, procedures and legal requirements. Ability to exercise initiative, judgment, discretion, and decision making consistent with organizational objectives. Proven ability to analyze and solve problems to deliver results.
Familiarity with standard office equipment. Ability to handle multiple tasks in timely manner.
Certifications, Licenses, Registrations:
Current licensed Oklahoma Licensed Professional Counselor (LPC), Licensed Marital and Family Therapy (LMFT), or Licensed Clinical Social Worker (LCSW). Additional licensure as a Licensed Alcohol and Drug Counselor (LADC-MH) or Licensed Behavioral Practitioner (LBP) preferred. American Heart Association BLS required;
