In addition to providing direct patient care, the Coordinator serves as the primary resource for coordinating workflow, facilitating communication, and ensuring patient care responsibilities remain on track throughout the shift.
RESPONSIBILITIES (ESSENTIAL FUNCTIONS):
Pre-shift planning includes but is not limited to reviewing patient assignments and census, identifying patients requiring increased assistance or higher-acuity care, determining bathing/oral care priorities for the shift, identifying anticipated admissions, discharges, and transfers, recognizing assignments that may require additional staffing support, and developing an organized workflow plan for the CNA/PCA team before shift report. This proactive planning allows the CNA/PCA team to begin their shift with clearly established priorities and promotes more efficient patient care throughout the day.
Conduct a brief huddle with the CNA/PCAs at 5:00 AM (night shift) or 5:00 PM (day shift). The purpose of the huddle includes but is not limited to reviewing completion of patient care responsibilities, identifying tasks that remain outstanding, prioritizing care that must be completed before the end of the shift, identifying barriers preventing completion of patient care, ensuring no patient care activities are unintentionally overlooked, and promoting teamwork and shared accountability during the final hours of the shift.
Following the huddle, the Shift Coordinator will assist with coordinating workload and assigning support for the remainder of the shift to help ensure all patient care responsibilities are completed safely and efficiently.
Serve as a communication resource for the CNA/PCA team by identifying trends, recognizing barriers to patient care, and promoting positive team performance. Examples of expected communication to the charge nurse or nursing supervisor includes but is not limited to workflow concerns affecting patient care, repeated barriers preventing task completion, staffing concerns or assignment imbalances, opportunities for process improvement, patterns of recurring issues requiring leadership awareness, and relaying outstanding teamwork or exceptional performance.
This communication is intended to provide nursing leadership with timely information regarding unit operations while ensuring concerns, recurring patterns, and team member recognition are appropriately elevated through the leadership chain.
Provides patient care, under the direction of the registered nurse (RN) or licensed practical nurse (LPN), which may include but is not limited to turning, moving, or ambulating patients, gathering medical supplies, bathing and grooming patients by brushing their hair, teeth, shaving them, etc., feeding patients and documenting their food and liquid intake, checking vital signs, finger sticks, answering patient call lights, cleaning rooms and bed linens, stocking supplies, assisting with some medical procedures, and transporting patients.
Accurately follow unit workflow guide listing duties to be performed during each shift.
May be required to perform as a Sitter. Sitter role may require floating to other units.
Protects patients and team members by adhering to infection-control policies and protocols.
Documents patient care services timely and accurately.
Maintains patient confidence and protects operations by keeping information confidential.
Ensures operation of equipment by verifying preventive maintenance requirements are completed, calling for repairs, and evaluating new equipment and techniques.
Maintains a cooperative relationship among health care teams by communicating information and participating in team problem-solving methods.
Regular attendance and punctuality for scheduled shifts.
Appropriately adapts assessment, treatment, and/or care methods to accommodate the physical, cultural, age-specific, and other developmental needs of each person served.
Maintains professional and technical knowledge through continuing education opportunities including internal and external educational offerings.
Must adhere to safety protocols at all times.
Utilization of assistive devices for lifting is mandatory.
Per DRH policy, all required conditions of employment must be met and maintained including required vaccinations.
