In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Easton, MD

Overview

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations. Evaluations are conducted in the member’s home and aim to bridge gaps in care with flexible, comprehensive, and personalized health assessments delivered directly to people’s front doors.

• Conduct in-home visits (approximately 45 minutes) with one-on-one engagement to answer health questions.
• Review medication and medical history, perform a physical evaluation, and, if ordered by the health plan, administer additional diagnostic tests (e.g., diabetic eye exams, spirometry).
• Utilize an iPad and a clinical workflow for each evaluation.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Be flexible to travel locally and within licensed states, as needed, and conduct virtual visits when required.
• Collaborate with Signify Health as a clinician colleague and fulfill the requirements of the specified role.
• Work toward visiting millions of people nationwide to connect them with appropriate care for their unique needs.

• Active, unrestricted licenses in the coverage area(s) (NP or PA).
• Board certification required.
• Multi-state licenses preferred.
• Anticipated weekly hours: 29.5

The primary workplace is the private residence of health plan members. Environments may vary and include households with pets, secondhand smoke, and other potential airborne allergens or irritants. Clinicians must be able to perform duties safely in home settings.
