- Technology comfort is essential — you’ll be working in an electronic health record, conducting some sessions by secure video, and using electronic consent and authorization workflows.
- Independent, not isolated — you’ll have a supervisor and a team of case managers to coordinate with, but the day-to-day pace is yours to manage.
The Targeted Case Management Community Support Specialist conducts comprehensive TCM assessments and develops individualized care plans for new referrals and required six-month reassessments. This role focuses on assessment, treatment planning, and the documentation, consents, and authorizations that keep our clients’ services active and compliant.
You will partner with our case managers, supervisors, and referral sources to identify each client’s needs and translate those needs into a clear, achievable plan. You will own the timelines for your own assessments and reassessments, the authorizations tied to them, and the consents and releases that make coordination of care possible.
This is a contractual position with flexible scheduling. You decide when the work happens — within the deadlines set by Maryland Medicaid, our Administrative Service Organization (ASO), and program requirements. Travel across our Eastern Shore clinics and into clients’ homes and communities is a regular part of the role.
Key Responsibilities Assessment and Care Planning- Conduct comprehensive TCM assessments for new referrals within 20 days of authorization.
- Complete six-month reassessments and updated care plans within required timelines.
- Evaluate each client’s strengths, needs, functional impairments, behavioral health concerns, and service needs.
- Develop individualized care plans with measurable goals, target dates, proposed interventions, and clearly assigned responsibilities.
- Incorporate input from the client, family or other supports (when authorized), and other treatment providers.
- Provide clinical recommendations and referrals based on each client’s needs and eligibility.
- Submit initial authorization requests to the ASO accurately and promptly after intake.
- Track each client’s authorization status, units used, and renewal dates so coverage never lapses.
- Submit reauthorizations and uninsured benefit renewals (typically every 90 days for uninsured clients) before they expire.
- Communicate authorization status to case managers and supervisors proactively — flag issues early, not after they’ve become problems.
- Maintain documentation that supports each authorization in case of audit.
- Obtain signed Agreements for Services, Consents to Treat, and Releases of Information at intake.
- Update consents and releases annually, or earlier when circumstances change.
- Document client confidentiality protections in accordance with HIPAA, 42 CFR Part 2 (when applicable), and Maryland state law.
- Track release expirations and revocations so the team always knows who we can and cannot speak with.
- Maintain accurate, timely, and confidential documentation of every assessment, reassessment, and care plan.
