- OASIS training preferred
- Must have valid driver's license and car for home visits
- Must be COVID vaccinated or have an exemption form on file
- Must have flu shot or declination, and mandatory mask wearing while at work

Description:

Provide physical therapy services to patients in their homes.
Assess patient needs and develop appropriate treatment plans.
Perform skilled visits with a minimum of 6 visits per day.
Utilize various equipment and devices effectively for patient care.
Provide patient and family education regarding home exercise programs.
Document patient progress and treatment plans accurately.
Work with patients suffering from cardiopulmonary, geriatric, neuro, ortho, and oncology conditions.
Work collaboratively with other healthcare professionals as needed.
Implement safety and injury prevention measures for patients.
Conduct reassessments and follow-up evaluations as required.
Familiarity with home health documentation systems is preferred.
Engage in continuous education to uphold treatment standards and methodologies.
