Careage Home Health is seeking Full-Time Licensed Practical Nurses (LPN) to serve patients throughout Thurston County.
Compensation & Schedule- $42-$47 / Depending on Experience
- Mileage Reimbursement
- Thurston County Service Area
- Monday-Friday

- Understands and adheres to established policies and procedures.
- Along with the team, implements the nursing care plan for each patient
- Ensure that proper Universal Precautions are practiced for resident and staff sanitation and safety
- Initiates preventive and rehabilitative nursing procedures as appropriate.
- Observes signs and symptoms and report to the physician and RN, reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
- As appropriate, prepares residents’ medications to accompany them on short-term absences from the community as allowed by the state.
- Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
- Maintains accurate and complete records of observations, treatments, and care of patients.
- Participates in medical record audit as assigned.
- Attends staff meetings, patient care conferences, and in-services as scheduled.
- Takes on-call duty, nights, weekends, and holidays as assigned.
- Submitting any changes in schedule to the Director of Clinical Services/Clinical Manager on a daily basis.
- Participating in patient care conferences to discuss the need for the involvement of other members of the health team, such as a physical therapist or speech-language pathologists.
- Prepares clinical and progress notes.
- Assists the physician and RN in performing specialized procedures.
- Prepares equipment and materials for treatments.
- Observes aseptic technique as required.
- Assists the patient in learning appropriate self-care techniques.
- Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
- Minimum of one (1) year experience in nursing, preferred.
- Acceptance of philosophy and goals of this Agency.
- Ability to exercise initiative and independent judgment.
- Familiar with Home Visit Protocol
Job Duties
- Proficient computer skills to manage the electronic health record (EHR)
- Ability to manage information electronically and transfer records appropriately
- Ability to work in an electronic environment
- Refined deductive reasoning skills to be able to problem solve on shift
- Must have valid driver’s license or dependable access to a vehicle for business use
- Must be in good health and physically/mentally capable of performing assigned tasks
- Working knowledge of personal computing and word processing software
- Proficient in Microsoft Windows
