Position Overview
Careage Home Health is hiring a Registered Nurse (RN) to provide home‑health services in the Olympia area of Thurston County. The role offers competitive wages ($50–$57 per hour, DOE), mileage reimbursement at IRS rate, and a supportive team environment.

- Full‑time benefits:

- Current Registered Nursing license in Washington state
- BLS Certification
- One (1) year of nursing experience (preferred)
- Home Health experience (preferred)
- Ability to travel within Olympia and surrounding Thurston County (required)
- Valid driver’s license and insured, reliable vehicle (required)
- Provides services in accordance with the plan of care
- Conducts initial evaluation visits and regularly reevaluates patient nursing needs
- Initiates plan of care and necessary revisions
- Provides services requiring specialized nursing skills
- Initiates appropriate preventive and rehabilitative nursing procedures
- Prepares clinical and progress notes for each patient visit and summarizes care conferences within Agency policy deadlines
- Coordinates services
- Informs personnel of changes in patient condition and needs
- Provides counseling to patients and family/significant others regarding nursing and related needs
- Participates in and presents at in‑service programs
- Adheres to established Agency policies and procedures
- Processes orders and notifies physician of patient needs and condition changes; completes certification/recertification orders and discharge summaries
- Determines the amount and type of nursing required for each patient
- Refers and coordinates care to Physical Therapist, Speech–Language Pathologist, Occupational Therapist, and Medical Social Worker for patients needing specialized services
- Supervises and trains other nursing personnel on the RN‑CM team
- Conducts patient care conferences for assigned patients; participates in peer review and quality assessment and performance improvement activities as assigned
