Memory Care Coordinator

Description

• Reviews and documents daily in the communication log as appropriate.
• Reviews new orders and EMAR daily.
• Supervises and reviews the transcription process, medication and equipment ordering, pharmacy cycle fill.
• Performs resident care coordination and communication with outside agencies and medical providers.
• Reviews alert charting.
• Reviews incident reports as needed.
• Reviews cleanliness and organization of medication room.
• Reviews, supervises and assists with resident care as needed.
• Performs medication pass.
• Assists residents ADLs.
• Reviews and documents on the service plan as needed.
• Attend service plans review as needed.
• Reviews and responds to Community IQR audits to ensure compliance.
• Assist and/or conduct the orientation for new team members.
• Schedule the exit-training interview, with new team members and review of skill checklist.
• Performs all other duties as assigned by any member of leadership.

• Keep all required licensure in good standing at all times.

• Ability to push, pull, and lift at least 50lbs.
• Ability to bend, squat, and climb stairs.
• Ability to continuously walk or stand.

• Potential exposure to blood and bodily fluids.
• Potential exposure to communicable diseases.
