Overview

Job Summary Seeking individual to support the Regional Oral Health and Health Insurance Literacy programs. Will provide education on health coverage and oral health, including insurance basics, enrollment, benefits and tax credits. Conducts outreach at health fairs and workshops and helps clients navigate coverage, care coordination and dental home placement. Maintains current knowledge of Medicaid, CHP+, Connect for Health CO, Co Connect & PEAK as well as insurance legislation and healthcare system changes.

Collaborates with partners and manages referral resources for primary and oral health services.

The Public Health Specialist II is responsible for planning, implementing, administering, and evaluating public health programs and initiatives that support community and population health objectives and disease prevention. Working directly with the public, community partners, and multidisciplinary teams, this position applies established public health principles and analytical methods to deliver programs, collect and analyze data, monitor program activities, support community engagement, and ensure programs are effective, compliant, and responsive to community needs across a variety of settings.

Responsibilities
  • Coordinate and carry out assigned public health program activities; coordinate meetings, trainings, and outreach with internal staff and community partners; maintain accurate and complete program records, documentation, and reports.
  • Track assigned work and identify issues or delays for review; work directly with the public and community partners through meetings, field activities, virtual visits, and, as appropriate, home visits to deliver services, support client skill development, assist with health system navigation, promote public health programs, and support community partnerships.
  • Support grant-funded activities by collecting documentation and supporting required reporting; participate in meetings, trainings, and work groups as assigned; adhere to applicable public health laws, policies, and ethical standards.
  • Independently implement and coordinate health education activities using approved materials and established guidance; support and deliver outreach and presentations; maintain required documentation in accordance with program procedures and public health policies.
  • Maintain awareness of relevant regional services and provide resources, referrals, and navigation to other public health programs and services and to outside agencies as appropriate.
  • Maintain accurate and organized records and data in accordance with established protocols and compliance with applicable laws, regulations and policies; protect confidentiality, security, and integrity of sensitive information.
  • Participate in the planning, implementation, and evaluation of public health programs and initiatives in accordance with policies and procedures.
  • Contribute to funding and grant applications and support contract or funding agreements by ensuring compliance with program guidelines, objectives, deliverables, and reporting requirements.
  • Assist supervisors and program leads with program development, monitoring, and quality-improvement activities; participate in programmatic data collection, disease surveillance, and reporting activities as required.
  • Maintain program records and databases to support monitoring, evaluation, and reporting; analyze data to identify trends, community needs, and opportunities for improvement.
  • Maintain awareness of relevant health and social services in the region and provide resource navigation and referrals to program participants and community members; identify and participate in community outreach activities.
  • Support health education and community health promotion activities designed to improve population health outcomes.
  • Establish and maintain effective working relationships with local health department staff, representatives of other agencies, community partners, stakeholders, and the public.
  • Collaborate with Communications staff to develop and disseminate program-specific messaging, educational materials, and outreach content; participate in meetings, coalitions, work groups, and partnerships as assigned.
  • Independently complete administrative and program support tasks, including inventory management, financial coding and expense tracking, maintaining records and databases, and preparing meeting notes.
  • Develop, update, and maintain program materials, tools, and documentation; ensure confidentiality of client and program information per HIPAA and policy requirements.
  • Perform duties with professionalism, cultural responsiveness, and adherence to ethical public health standards; perform other duties as assigned.
  • Regular and predictable attendance is required.
