Case Manager - La Plata County Jail or Durango Integrated Healthcare, Durango, CO

Position Summary:

The Case Manager participates as a member of the Jail-based program to provide intensive case management support to patients by initiating services while incarcerated and to transition services into the community. This position is part of a multidisciplinary team that provides services inside detention facilities designed to assist and support individual's diagnosed with or being assessed for a mental health or substance use disorders, to gain access to needed medical, social, educational, and other services, as well as provide care coordination and care transition services.

Qualifications:

Our ideal candidate for our Case Manager position will have a bachelor’s degree in one of the recognized mental health professions.

They will also have had previous experience with substance abuse and mental health related case management and treatment, and one-year or more human service-related experience preferred.

Salary:

Starting pay is $21.00 (Based pay $18.50 + $2.50 Locale Differential = $21.00) and ranges up to $23.72 per hour (which includes a $2.50 locale differential at our Durango worksites) and considers preferred experience and preferred education described above.
