Community Collaboration Coordinator for La Plata County

No single organization can meet the needs of children, youth, and families alone, it takes coordinated effort, shared trust, and consistent follow-through. As the Community Collaboration Coordinator for La Plata County, you play a vital role in making that coordination possible across La Plata County, providing dedicated leadership, relationship-building, and operational support to coordinate multi-sector partners and initiatives. You'll coordinate the day-to-day work that keeps collaboration moving forward: facilitating meetings, building relationships across sectors, holding strategies and action plans, and coordinating initiatives to advance youth mental health across the county.

Team Up unites and supports local organizations and individuals in addressing community-identified needs to help people thrive in Southwest Colorado. Team Up serves five counties in the southwest region through community service, community support, and community collaboration.

Team Up's Community Collaboration Program brings together individuals and organizations across Southwest Colorado to address deeply seeded issues and emerging opportunities through a data driven and informed Collective Impact framework. Over 60 entities across Southwest Colorado have stepped up to work together to create a thriving community for children, youth, and families. Team Up partners participate in Action Teams, meeting on a regular basis to collaborate on specific topic areas that have been identified and are driven by the community.

Additionally, each county has a Steering Committee that helps steer the overall direction of Team Up's collaborative work.

Team Up is the backbone (a team dedicated to aligning & coordinating the work of the group) for this Collective Impact Initiative to support these collaborative efforts. Backbone efforts include but are not limited to:

  • Cultivate community engagement and ownership
    • Inclusively and authentically engage. Ask "who isn't here?" Seek and include missing voices
    • Promote collective ownership through member agency and decision making
  • Mobilizing resources
    • Fundraising and grant proposal writing
    • Recruit and include key stakeholders
  • Coordination
    • Schedule meetings, prepare agendas, send reminders, encourage accountability, facilitate meetings
    • Support aligned partner activities
    • Build relationships among members
  • Guiding vision and strategy
    • Create focus around collaborative priorities and goals
    • Action and strategic planning (and keeping teams on track)
  • Measurement
    • Establish shared measurement toward goals
    • Data collection and management

Through these continuous backbone efforts, Action Teams are able to consistently meet and move projects forward in a constructive way. Action Team members are the drivers of strategy, action, and decisions that will ultimately impact the wellbeing of the population in which they are aiming to serve.

Primary Job Responsibilities/Duties

  • Facilitation & Coordination of Steering Committee and Action Teams
    • Facilitate and coordinate all monthly steering committee, action team, subcommittee, and partner convenings, including agenda development, meeting logistics, note-taking, and follow-up. Current teams include:
      • La Plata County Steering Committee
      • Transportation Action Team
      • Scholarship Action Team
      • Mental Health Action Team
    • Facilitate and coordinate quarterly county-wide convenings focused on youth mental health
    • Build and maintain a productive, inclusive group culture across diverse partners
    • Develop and distribute meeting follow up, notes, action items trackers, and shared resources
    • Track progress on team member commitments and support accountability across partner organizations through regular check-ins and follow up
    • Coordinate scheduling, venues, technology, accommodations, and materials for meetings and events
    • Maintain records and documentation required for grant compliance and reporting
    • Coordinate and facilitate new member orientation as new members join initiatives
  • Partnership & Relationship Development
    • Serve as a primary point of contact for partners within La Plata County
    • Cultivate and maintain strong, trust-based relationships with community partners, service providers, agencies, schools, and community members to build support for and to advance Team Up work
    • Identify and engage new partners to strengthen the initiative's reach and impact
    • Represent the initiative in community settings, coalitions, and public forums to build relationships, maintain a full understanding of the current landscape of local and regional activities, and align their work into Steering Committee and Action Team…
