Community Connections seeks a part-time Community Engagement Coordinator to support Southwest Colorado Cares (SWCC), a regional resource hub designed to help community members and partner organizations identify and access local supports that address social drivers of health. This position coordinates and supports the Community Ambassador program, including recruitment, training, coordination, outreach, and program improvement. The Coordinator also supports maintenance and improvement of the SWCC/211 resource directory so that information is accurate, accessible, culturally responsive, and useful to community members and partner organizations across southwest Colorado.
This role requires strong relationship-building, communication, organizational, and research skills, as well as experience in community engagement, human services, public health, or related fields. A degree in social work, social sciences, human services or related field and/ or four years of relevant experience is required. This is a part-time hybrid position, $22.10/hr.

Status: Part-time, Temporary through June 30, 2027 with potential for one-year extension. 15-20 hours a week.
Hourly Rate: starting at $22.10
Location: Hybrid. This position can be based out of any Community Connections location in southwest Colorado or remote elsewhere in Colorado.
Job Description: The Part-Time Community Engagement Coordinator supports Southwest Colorado Cares (SWCC), a regional resource hub designed to help community members and partner organizations identify and access local supports that address social drivers of health. This position coordinates and supports the Community Ambassador program, including recruitment, training, coordination, outreach, and program improvement. The Coordinator also supports maintenance and improvement of the SWCC/211 resource directory so that information is accurate, accessible, culturally responsive, and useful to community members and partner organizations across southwest Colorado.

Community Connections, Inc. (CCI) connects people with disabilities, developmental delays, and complex needs to the resources they choose to lead healthy and fulfilling lives. Southwest Colorado Cares expands this mission by strengthening regional access to reliable information, referrals, and community-based supports. The Community Ambassador program includes community members with lived experience and trusted community connections who help promote SWCC, gather feedback, and ensure the resource hub remains responsive to local needs.
Key Responsibilities- Coordinate and improve the SWCC Community Ambassador program, including program framework, goals, timelines, documentation, and implementation processes.
- Recruit and train Community Ambassadors as needed from target communities, including Spanish-speaking community members, older adults, people with disabilities, caregivers, and other populations experiencing barriers to services.
- Maintain Ambassador training materials that explain SWCC, the referral process, outreach expectations, communication guidelines, feedback collection, and resource directory use.
- Support Ambassadors in community outreach, education, event participation, and collection of community feedback about resource needs, barriers, and service gaps.
- Build and maintain relationships with community partners, local organizations, service providers, faith-based groups, clubs, senior groups, and community associations to support the resource directory and the Community Ambassador program.
- Communicate the mission and role of SWCC and the Community Ambassador program to community members and partner organizations.
- Support the SWCC/211 resource directory by helping research, verify, update, and organize resource information for the five southwest Colorado counties.
- Maintain relationship with Mile High United Way and TeamUP 211 community liaison to support the resource directory and partnership development.
- Document partner organization information, Ambassador activities, outreach results, feedback, and resource directory updates in internal systems as needed.
- Create promotional and educational materials that increase public awareness of SWCC, the Community Ambassador program, and the regional resource directory.
- Maintain confidentiality and use appropriate discretion when handling community member, partner, or organizational information.
- Participate in staff meetings, partner meetings, trainings, and other program-related activities as assigned.
- Perform additional duties as assigned by the Senior Director.
- Four years of relevant experience in community engagement, human services, social services, public health, outreach, program coordination, case management, or a related field; OR
