This position is for the Back Home SSVF (Supportive Services for Veteran Families) program which is a Rapid Rehousing program that utilizes a 90‑day model to find and stabilize housing for veterans experiencing homelessness or at risk of homelessness. Primarily focusing on housing case management, the role also offers wrap‑around case management services assisting veterans to connect to Public Benefits, VA benefits, health referrals, and financial health referrals.
Job SummaryThe Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. Duties include conducting needs‑based and program eligibility assessments, engaging in community outreach, providing case management services, and coordinating program‑wide services. The case manager is expected to demonstrate progress toward applying field‑relevant best practices and standards.
Essential Duties And Responsibilities- Provide case management services to program clients.
- Identify and serve the unique needs of participant households using screening and assessment tools to ensure completion of needs‑based screenings.
- Understand and implement the assigned program according to program funder and VOA standards and expectations.
- Conduct community‑wide outreach to identify eligible participant households presenting with substantial barriers to housing stability.
- Provide group educational and other activities for current or prospective program clients as assigned.
- Support team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field.
- Attend orientations, trainings, educational programs, staff meetings, community meetings, conferences, and workshops as requested and applicable.
- May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices.
- Promptly and clearly document all client interactions along with required eligibility and demographic information using designated databases.
- Perform duties professionally while maintaining confidentiality of all information and participating effectively within and across teams.
- Participate in professional development activities to promote the application of field‑relevant best practices and standards, making demonstrable progress toward these practices.
- Perform all other duties as assigned.

- Bachelor’s degree in human services, social work, or a closely related field, or related experience.
- One year of direct experience in the provision of human services.
- Must possess a Colorado driver’s license and state‑mandated automobile insurance.
- Must possess a personal vehicle that may be used for work‑related travel (reimbursement for mileage is available).
- Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.
- SOAR (SSI/SSDI Outreach, Access, and Recovery) Certification.
- Knowledge and skill in the application of Harm Reduction, Critical Time Intervention, Motivational Interviewing, Trauma‑Informed Care, and Housing First Principles.
