Position Overview

The Case Manager will support the Back Home SSVF (Supportive Services for Veteran Families) rapid rehousing program, a 90‑day model designed to find and stabilize housing for veterans experiencing homelessness or at risk of homelessness. The role focuses on housing case management and connects veterans to public benefits, VA benefits, health referrals, and financial health referrals.

Responsibilities
  • Provides case management services to program clients.
  • Identifies and serves the unique needs of participant households using screening and assessment tools.
  • Understands and implements the assigned program according to funder and VOA standards.
  • Conducts community outreach to locate eligible households with substantial housing barriers.
  • Leads group educational activities for current or prospective clients.
  • Supports team members as a resource for problem analysis and knowledge sharing.
  • Attends orientations, trainings, staff and community meetings, conferences, and workshops as required.
  • Provides training and mentorship to team members and community stakeholders on best practices.
  • Documents all client interactions, eligibility and demographic data in the designated databases.
  • Performs duties professionally, maintaining confidentiality and effective teamwork.
  • Engages in professional development to improve field‑relevant best practices.
  • Performs additional duties as assigned.
Requirements
  • Bachelor’s degree in human services, social work, or a closely related field, or equivalent experience.
  • One year of direct experience providing human services.
  • Colorado driver’s license and state‑mandated automobile insurance.
  • Personal vehicle for work‑related travel (mileage reimbursement available).
  • Complete agency and program credentialing within 12 weeks of hire and maintain standards thereafter.
Preferred Qualifications
  • SOAR (SSI/SSDI Outreach, Access, and Recovery) Certification.
Knowledge and Skills
  • Knowledge and skill in Harm Reduction, Critical Time Intervention, Motivational Interviewing, Trauma‑Informed Care, and Housing First Principles.
  • Ability to respectfully serve individuals from diverse backgrounds, cultures, ideologies, and religions.
  • Ability to work and thrive within a diverse, multicultural team environment.
  • Ability to take initiative and work independently.
  • Effective verbal and written communication skills.
  • Ability to apply self‑care strategies in challenging or traumatic situations.
Working Conditions and

Physical Requirements
  • Regular travel within the program’s service area.
  • Work in diverse environments such as homeless shelters, service facilities, streets, offices, and other locations as needed.
  • Physical ability to bend, climb, stoop, kneel, reach, crouch, squat, lift 30–50 pounds, balance, stand, sit, walk, and perform fine motor tasks. Requires visual and auditory acuity.
Position Type and Hours
  • Full‑time; 40‑hour workweek with occasional overtime.
  • Potential for hybrid work.
Salary
  • $22.00 – $24.00 per hour, based on experience.
