SummaryThe Service Coordinator is directly responsible for the service provision and housing assistance for the residents in the Affordable Housing Portfolio. This program serves seniors and persons with disabilities located in the affordable housing sites. Service modalities include Trauma Informed Care, Harm Reduction, and Motivational Interviewing. The position works in conjunction with the Community Administrator at the housing site. Job duties include conducting needs‑based assessments, community outreach, providing case management services, supporting housing retention, connecting residents to income resources and/or public benefits, attending meetings/trainings within the community, and collaborating with service providers and multi‑disciplinary teams to enable residents to live independently and age in place.
The coordinator also develops and facilitates health, wellness, and educational programs tailored to resident needs to improve wellbeing and foster a strong sense of community. The role maintains existing partners and establishes new partnerships to expand opportunities and resources available to residents.
- Ensures that the resident’s voice is heard, and their ideas play a prominent role in programming.
- Within program guidelines, facilitate and accept referrals on residents from the Community Administrator, family members, service providers, and the residents themselves.
- Assess services needed by residents in the building through surveys, visitations, group meetings, etc. Complete an assessment and collect needed information. Identify and serve the unique needs of each resident using screening and assessment tools.
- Facilitate the delivery of appropriate services for households as indicated by relevant service modalities through an equity lens.
- Provide residents with information on specific services and/or program providers, giving several options whenever possible. Comply with all Fair Housing requirements.
- Encourage residents and/or family members to arrange for required services when possible to maintain and encourage independence.
- Coordinate and monitor the services and activities of providers to residents to assure quality of care and cost effectiveness. Advocate for residents per their wishes.
- Maintain up‑to‑date files on service providers in the community by using and becoming knowledgeable with the United Way database and Benefits Check‑up.
- Maintain and submit accurate and updated records on all residents, including the services they receive.
- Develop support groups and workshops in response to needs and facilitate or co‑facilitate these groups on a regular basis.
- Assist in relocating residents as needed and securing emergency housing.
- Support team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field.
- Responsible for implementing the Service Coordinator program in accordance with HUD and VOA guidelines.
- Attend orientations, trainings, education programs, staff meetings, community meetings, conferences, and workshops as requested and applicable to meet the needs of the position. May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices.
- Clearly document all client interactions along with required eligibility and demographic information.
- Perform duties in a professional manner by maintaining the confidentiality of all information and by participating effectively within and across teams.
