Job Description

Job Purpose or Objective(s): You will report to the Presiding District Court Judge and manage the efficient delivery of case management duties for the Healing to Wellness Court staff and judges observing all relevant procedures.

Monday-Friday | 8:00am-4:30pm | Hybrid work is a possibility after the first 90 days

Primary Tasks:

  • Conduct client investigation, intake, orientation, and assessments to determine treatment and recovery needs.
  • Develop, implement, and monitor individualized treatment and wellness plans.
  • Coordinate chemical dependency evaluations and appropriate treatment services.
  • Facilitate individual and group counseling sessions aligned with treatment goals.
  • Provide substance abuse education and promote recovery principles, including Wellbriety and Choctaw recovery programs.
  • Monitor client participation, compliance, and progress throughout treatment.
  • Collaborate with clinical staff, supervisors, families, and support systems to support client success.
  • Coordinate referrals and connect clients with community resources and support services.
  • Organize educational, cultural, spiritual, and recreational activities that support holistic recovery.
  • Develop relapse prevention, aftercare, discharge plans, and maintain required case management documentation and records.
  • Perform other duties and responsibilities as may be assigned.

Minimum Requirements:

· Bachelor's degree in human services/mental health, criminal justice, social work, or another related field or four (4) years of directly related experience.

· Licensed Addictions Counselor or Certified Addiction Counselor.

· Alcohol and Drug Counselor trainee status or ability to obtain within the 90-day probation period.

· Knowledge of individual case planning, case management, and working with individuals in a helping capacity.

· Excellent case and file management skills

· Knowledge of available Choctaw Nation and community resources.

· Knowledge of Healing to Wellness Court goals and strategies, violence and addiction reduction initiatives.

· Demonstrate a high level of cultural competence, sensitivity, and responsiveness.

· Must have a valid driver's license and be able to pass a criminal background check.
