Duke Health Integrated Practice comprises more than 110 primary and specialty outpatient clinics, extending the reach of the Duke Health mission across the state of North Carolina.
JOB SUMMARYPerform outreach, community engagement, and administrative support duties under supervision as part of a grant-funded pregnancy and child health initiative. This position supports activities such as Centering Pregnancy and Parenting group sessions, assists with program coordination and administrative tasks, and provides outreach and resource assistance to patients and families within the community. The role requires fluency in Spanish, strong communication and organizational skills, and the ability to connect effectively with diverse.
JOBDUTIES AND RESPONSIBILITIES
- Conduct outreach, education, and care management activities under supervisory guidance.
- Facilitate or assist with Centering Pregnancy and Parenting group sessions to promote health engagement, prenatal education, and family support.
- Assist with administrative functions, including scheduling, data entry, maintaining tracking forms, and filing program documentation.
- Educate clients about community and health resources; assist with accessing services as needed.
- Advocate for clients and support development of self-care and problem-solving skills.
- Accompany clients to appointments or community resource sites as appropriate.
- Maintain accurate and timely records of activities, visits, and outcomes.
- Provide feedback on barriers to service accessibility and opportunities for program improvement.
- Support other program-related tasks and administrative duties as assigned.
- Work requires an educational background equivalent to a high school diploma or GED. Additional training or certification as a Community Health Worker preferred.
- Work requires at least one year of experience in a position involving patient or public contact.
- Requires fluency in Spanish (speaking, reading, and writing).
- Skills:
- Bilingual (English/Spanish) fluency.
- Strong communication, organizational, and interpersonal skills.
- Ability to work collaboratively with clinical and community partners and independently as needed.
- Proficiency with basic office software (e.g., Microsoft Excel, Word) and data management tools.
