Purpose Of PositionAxil Health is a privately owned pharmacist collective offering independent pharmacy, consulting, and clinical services located inside established medical facilities. The Pharmacist Fellow role plays a pivotal role in Axil Health’s patient care and chronic disease management clinical model. This role is dedicated to providing exceptional patient care, focusing on chronic disease management, patient care coordination, and adherence monitoring. Additionally, the Pharmacist Fellow will contribute to cutting‑edge research initiatives, evaluating disease states, and assessing new pharmaceutical products or treatments.
MajorResponsibilities & Accountabilities
- Patient Care Management:
- Provide comprehensive medication management and counseling to patients with chronic diseases, ensuring optimal therapeutic outcomes.
- Collaborate with healthcare providers to develop and implement individualized treatment plans tailored to patients’ needs.
- Utilize remote patient monitoring technologies to track patient progress and intervene proactively to prevent complications.
- Chronic Disease Management:
- Develop protocols and guidelines for the management of chronic diseases, including diabetes, hypertension, cardiovascular diseases, and others.
- Monitor patient adherence to treatment regimens and adjust therapies as necessary to achieve therapeutic goals.
- Educate patients on disease self‑management strategies and lifestyle modifications.
- Research and Evaluation:
- Conduct literature reviews and analyze scientific literature to stay abreast of advancements in disease states and treatment modalities.
- Design and implement research studies to evaluate the efficacy and safety of new pharmaceutical products or treatment approaches.
- Collect and analyze data, interpret results, and prepare research findings for publication or presentation at conferences.
- Collaboration with Healthcare Providers:
- Collaborate with physicians, nurses, and other healthcare providers to ensure coordinated and integrated patient care.
- Participate in multidisciplinary rounds, case conferences, and team meetings to discuss patient cases and treatment plans.
- Serve as a resource to healthcare providers regarding medication management, drug interactions, and therapeutic alternatives.
- Adherence Monitoring:
- Implement strategies to assess and improve medication adherence among patients, including the use of adherence aids and patient education.
- Monitor adherence patterns and intervene when necessary to address barriers to adherence and optimize treatment outcomes.
- Doctor of Pharmacy (Pharm.
D.) degree from an accredited pharmacy program. - FAPhA accreditation.
- Active pharmacist licensure in the appropriate state(s).
- Board certification in ambulatory care pharmacy (BCAP) or other relevant specialty certification is desirable.
- Strong clinical knowledge and understanding of chronic disease management principles.
- Excellent communication, interpersonal, and teamwork skills.
- Ability to thrive in a fast‑paced clinical environment and manage multiple priorities effectively.
- Proficiency in utilizing electronic health records (EHR) and pharmacy management systems.
- Commitment to evidence-based practice and continuous professional development.
- Ability to use a keyboard to enter and retrieve data.
- Must have good eyesight to view computer monitor and phone.
- Lifting and/or carrying of 5 to 20 lbs.
- Willingness to travel.
