Chronic Disease Specialist

• Provide technical assistance, guidance, and subject matter expertise to regional Rural Organizations
• Support program planning and execution across cancer prevention and control, early detection and screening
• Analyze chronic disease trends
• Advise on evidence-based clinical and community strategies
• Strengthen provider engagement and multidisciplinary care models
• Coordinate Hub data collection, monitoring, and interpretation related to cancer and tobacco outcomes

• Bachelor's degree from an appropriately accredited institution
• Three (3) years progressively responsible experience related to the area of assignment
• Demonstrated experience applying public and community health principles to chronic disease, cancer control, or tobacco/nicotine cessation initiatives
• Proven skill delivering training and technical assistance to community or regional stakeholders
• Applied experience collecting, validating, and analyzing cancer-or tobacco-related data
• Demonstrated effectiveness communicating technical guidance to program stakeholders
• Documented experience leading or supporting initiatives aimed at reducing cancer burden or decreasing tobacco/nicotine use

Demonstrates expertise in public and community health principles, particularly in chronic disease management, cancer control, and tobacco cessation. Proven ability to analyze data, provide technical assistance, and communicate effectively with stakeholders to support program planning and execution.
