Job Summary

The Heart Center Communications Department at Duke University Hospital delivers exemplary care for heart patients to ensure an excellent patient experience, every time. The call center receives an average of 200 calls per day. Required skills include telephonic care management, triage, messaging, scheduling, prescription refills, medication reconciliation, patient education, motivational support, and identification of potential readmission risks.

Responsibilities
  • Apply expert‑level clinical decision‑making based on Benner’s Novice to Expert Nursing Theory to triage multiple demands and determine patient acuity.
  • Provide guidance to internal and external customers to facilitate safe patient and operational needs, anticipating safety concerns and identifying high‑risk patient situations.
  • Engage in excellent written and oral communication, uphold professionalism, and maintain excellent phone etiquette.
  • Navigate complex systems and processes to maintain patient‑centered care.
  • Collaborate with members of the care team (internal and external) to facilitate patient care needs.
  • Communicate, coordinate, and schedule tests and pre‑procedures for Electrophysiology (EP) procedures, including pacemakers, defibrillators, ablations, Watchman implants, etc.
  • Provide telephonic care management to ensure safe, patient‑centered care; assess cardiac patients via phone, triage, schedule, and educate for cardiac procedures.
  • Coordinate scheduling for procedures such as Dx & Intervention Cardiac Cauterization, Left Heart Catheterization, Right Heart Catheterization, TAVR, Mitra‑Clip, Heart Biopsy, Pre‑Lung Transplant Evaluation, Balloon Pulmonary Angiography, Peripheral Arteriograms, ASD & PFO Closure, and more.
  • Deliver transitional care: follow‑up of patients from hospital to home, assess discharge plans, perform medication reconciliations, and provide continued patient education.
  • Manage prescription refills for Duke University Hospital Cardiologists and perform other duties incidental to the work described.
Knowledge, Skills, and Abilities
  • Maintain current knowledge of the Emergency Medical Treatment and Labor Act (EMTALA) and nursing scope of practice.
  • Apply nursing processes and record activities, document outcomes, plans of care, interventions, and ongoing patient evaluations.
  • Seek resources independently and work collaboratively with teams.
  • Establish and maintain effective working relationships.
  • Communicate clearly in written and oral formats.
  • Integrate software and new technologies into work processes.
  • Understand procedures, resources, and techniques involved in administering routine and specialized treatments to patients.
  • Remain focused and organized in high‑demand environments.
Minimum Qualifications Education
