Transplant Coordinator – Duke University HospitalDuke University Health System – Duke University Hospital seeks to hire an experienced Transplant Coordinator who will embrace our mission of Advancing Health Together.
Department ProfileThe Post Lung Transplant Department at Duke University Hospital is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time. The transplant coordinator role has direct patient contact in education and assessing potential transplant candidates in clinic and virtual settings.
Responsibilities- Educate patients and families regarding all aspects of transplantation care, including arranging follow‑up appointments, labs, and coordinating with the multidisciplinary team during the post‑transplant phase.
- Communicate with patients and caregivers regarding the plan of care and ongoing changes.
- Maintain a caseload of post‑transplant patients to ensure timely care.
- Provide continuity of care between dialysis, outpatient care, and long‑term management.
- Make clinical judgments based on synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.
- Collaborate with medical faculty, pharmacy, nursing personnel, case management and other health disciplines to facilitate meeting patient and family needs.
- Stay current on transplant research and apply evidence‑based recommendations to patients/families.
- Complete regulatory documentation in the UNOS database and hospital EMR.
- Review education materials and work with the department on changes in current practice.
- Manage immunosuppression plan per protocol/medical team; provide ongoing education with patients and caregivers.
- Share on‑call responsibilities with a large team for organ offers for lung transplant candidates.
- BSN required; MSN preferred.
- 5 years of nursing experience with at least 2 years in transplant, critical care, or disease‑specific care.
- Current or compact RN license in the state of North Carolina; BLS required.
- Ability to multitask and prioritize multiple demands.
- Patient‑centered and family coaching skills.
- Ability to work in a high‑stress environment.
Full‑time:
Day shift, on call with some weekends/holidays.
