Position Overview

Carver Rehabilitation and Living Center is seeking an experienced, detail-oriented MDS Coordinator (RN/LPN) to oversee and manage our facility’s assessment and care planning processes. In this vital role, you will ensure compliance with all federal, state, and local standards while optimizing clinical outcomes and reimbursement accuracy.

Key Responsibilities
  • Assessment Management: Monitor, guide, and ensure the timely completion of all MDS assessments.
  • Data Transmission: Oversee and supervise the inputting and electronic transmission of MDS assessments to the state, ensuring successful receipt of validation.
  • Care Planning: Schedule, facilitate, and manage comprehensive resident care plan conferences.
  • Quality Metrics: Facilitate monthly Quality Measure (QM) meetings to promote continuous facility improvement.
Qualifications and Requirements
  • Education: Must hold a Nursing Degree from an accredited college or university.
  • Licensure: A current, unencumbered, and active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in North Carolina (or compact state) and active CPR certification.
  • Experience: At least 2 years of dedicated experience in Long-Term Care (LTC) as an MDS Coordinator.
  • Certification: RAC-CT certification is highly preferred. However, we will sponsor and offer RAC-CT certification training for experienced MDS Nurses looking to upgrade their credentials!
Key Skills and Qualities
  • Deep, outstanding knowledge of Case-Mix, Federal Medicare PPS processes, and Medicaid reimbursement.
  • Solid, current understanding of federal, state, and local standards, regulations, and guidelines governing long-term care facilities.
  • Excellent written communication, verbal communication, and critical thinking abilities.
  • Exceptional organizational skills with the proven ability to prioritize tasks effectively in a fast-paced environment.
