Day shift (8-4:30pm Monday-Friday); no weekends, call or holidays required.
Department Profile: The Abdominal Transplant Programs at Duke University Hospital is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time. The Abdominal Float Transplant Coordinator will work with adult and pediatric (inpatient and outpatient) transplant patients followed by the kidney, pancreas, liver, intestine and living donor programs.
The Transplant Coordinator will provide care for patients in the referral, evaluation, and acute transplant process, and post-transplant, pre and post donation of kidney or liver segment. The role floats to the various programs to provide care as a transplant coordinator. This position does extensive patient assessment in clinic environment, phone triage, and patient education to ensure exemplary care.
The Abdominal Float Transplant Coordinator provides follow-up and care across the continuum from pre-transplant, during transplant and post-transplant. S/he collaborates with members of the health care team to enhance patient care by developing, supporting and educating nursing staff. S/he is a clinician, educator, investigator, consultant and leader. S/he makes clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.
The Abdominal Float Transplant Coordinator is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, teaching, and in providing leadership to the organization.
