Case Manager; RN; Home Health

Role Overview: A registered nurse responsible for delivering comprehensive, patient-centered care in home and inpatient hospice settings across multiple counties.

• Provide and document nursing care using the nursing process, ensuring compliance with regulations and standards.
• Perform assessments, develop personalized care plans, and collaborate with interdisciplinary teams.
• Educate patients and families on disease management, medications, and care strategies to promote positive outcomes.
• Coordinate care activities, including case management, discharge planning, and OASIS assessments.
• Maintain accurate documentation and adherence to safety, quality, and regulatory standards.

• Associate Degree or Diploma in Nursing; BSN preferred after two years.
• Current RN license in North Carolina or a compact state.
• CPR certification (American Heart Association).
• Valid driver's license with current automobile insurance.
• 1-2 years of med-surgical, critical care, or home health experience preferred.
• Strong assessment, critical thinking, communication, and documentation skills, with ability to work independently.
