The Duke Home Care & Hospice Care Coordinator coordinates home health, hospice, and infusion services across Duke Health facilities, including inpatient and outpatient settings. You will educate patients and families about infusion devices and work with the interdisciplinary team to ensure seamless care.
Required BSN, NC or compact RN licensure, and BLS, with at least three years of nursing experience. Duties include electronic documentation and rotating shift coverage across Duke sites.
