JOB SUMMARYThis position performs professional social work to individuals and/or families with personal, social, health and economic needs. Work in this class involves providing intensive social work services in serious and complicated cases which require the flexible use of a wide range of social work skills and intervention techniques. This work includes going into the adult’s home, assessing the situation and evaluating their individual needs.
Work may involve providing in-depth assessment of individual and family dynamics and needs, assessment, intervention and treatment of patients and/or families with acute to severe medical and/or emotional disorders and mental illnesses, or advance social work intervention, and instruction. Work is distinguished from the Social Worker II classification by greater involvement in sensitive and traumatic situations such as removal of a client from their homes, catastrophic and terminal illness, and/or family dysfunction.
Employees supervise and train lower-level social workers, students, and interns. Employees typically report to a Social Work Supervisor II under general supervision. Employees may have combined investigative and treatment roles or may have separate functions depending on the size and organizational structure of the departments of Social Services. An incumbent in this position may be designated “Social Worker I”, “Social Worker II”, “Social Worker III”, or “Social Worker Trainee”.
DUTIES
- Address client needs; develop strategies for delivery of appropriate services to clients.
- Educate the caregiver of children, elderly and disabled adult community about the CAP/DA waiver.
- Submit the service request and referral for individuals seeking to participate in the CAP/DA Waiver.
- Provide help in obtaining documentation from medical staff to monitor ongoing care needs.
- Access a beneficiary on an annual basis to determine ongoing appropriateness for CAP/DA services.
- Provide case management to the CAP/DA waiver beneficiary.
- Ensure waiver and non-waiver services are accurately listed on the plan of care initially, annually and during a plan of care revision.
- Provide regular and routine beneficiary monitoring to assure health, safety, and wellbeing, quality assurance reporting and beneficiary risk mitigation.
- Maintain confidential records and files and prepare reports as required.
- Assist and support clients in their use of agency and other resources.
- Refer beneficiaries to related agencies as appropriate.
- Attend scheduled staffing with supervisor and unit meetings
- Attend training sessions as available.
- Perform other related duties as assigned, including disaster shelters and possible APS on-call.
- Knowledge of methods and principles of casework and training.
- Knowledge of social work principles, techniques, and practices and their application to specific casework and community problems.
- Knowledge of behavioral and socioeconomic problems and their treatment and governmental and private organizations and community resources.
- Knowledge of the laws, regulations, and policies which govern social work programs.
- Skill in expressing ideas clearly and concisely and in planning and executing work effectively.
- The Social Work Supervisor II assigns work in terms of very general instructions. The supervisor spot-checks completed work for compliance with procedures and the nature and propriety of the results.
