Position: Case Manager II - Waiver Case Management (Twin Cities/St. Louis County)

Case Manager II

We are seeking a Case Manager II to work on our Waiver
Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

We offer work-life balance with reasonable case loads of an average of 30.

This Case Manager will be based out of the Twin Cities metro area, while supporting our St. Louis clients that reside in the Twin Cities area and Central Minnesota.

Hours: Full-Time, Monday-Friday, business hours 8am-4pm

Location: St. Louis County, Minnesota | Hybrid position

Travel: Some travel required to St. Louis County clients.

Hiring Range: $24.25 - $25.75/ Hourly, based on qualifications

Hiring Bonus: $1,000 Hiring bonus to external candidates

Job Duties
  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
