Case Manager II – Targeted Case Management (Dakota County)

Hourly Pay: $24.30 - $25.35, based on qualifications
Hiring Bonus: $1,000 to external candidates
Responsibilities- Must be able to have onsite presence within Dakota County, Minnesota
- Attend bi‑annual meetings with clients to monitor services
- Develop and maintain a collaborative relationship with clients, serving as a role model for clients by demonstrating positive, proactive and respectful communications
- Assess client needs and ensure continuity of care, which may include outreach and consultation with other partners and providers
- Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed
- Make referrals and authorize services based on needs
- Maintain records and document all coordination activities
- Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards
- Passing all background and requirements for facility and position
- Ability to pass the screening required by Federal Human Services Office of Inspector General to assure that the person has not been excluded from working in the Health Care Programs which receive federal funding
- Must have a valid and current driver’s license and auto insurance
- Valid driver’s license, excellent driving record and current auto insurance
- Bachelor’s Degree in behavioral science or related field
- 2,000 hours of supervised experience working with adults with severe and persistent mental illness
- OR Master’s Degree in behavioral science or related field
- Experience providing direct services to adults with serious and persistent mental illness
- Experience with case management and commitment proceedings
- Knowledge of Recovery, Motivational Interviewing and Person‑Centered approach
- Accrue up to 4 weeks of PTO/ESST, plus company‑designated holidays and floating holidays
