All Care Companions LLC is looking for a compassionate and dedicated In Home and Community Care Support Individual to join their team in Eagan, MN. Responsibilities- Provide direct, hands‑on personal care and daily living assistance to clients in home and community settings
- Support clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparation
- Accompany and assist clients with community outings, appointments, and social activities
- Monitor and document client health, behavior, and progress, reporting changes to supervisors
- Implement individualized care plans tailored to each client's unique needs and goals
- Foster a safe, supportive, and dignified environment for all clients
- Build trusting relationships with clients, families, and care team members
- Schedule:
8:00am‑2:00pm M‑F - Family prefers female caregiver and dog friendly person
- Prior experience in home care, personal care assistance (PCA), direct support, or a related caregiving role preferred
- Genuine compassion, patience, and dedication to supporting individuals with diverse needs
- Ability to pass a background check as required by Minnesota state regulations
- Valid driver’s license and reliable transportation preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a collaborative care team
