Case Manager II - Waiver Case Management (Full Time, $24.25-$25.75 hourly)
Location:
PEOPLE INCORPORATED MAIN OFFICE – EAGAN, MN 55121;
Hybrid in St. Louis County and surrounding Minnesota
Hours:
Monday-Friday 8:00 a.m.
-4:30 p.m.;
Full-Time;
Works two hybrid shifts after initial training
Hiring Bonus: $1,000 to external candidates
Responsibilities- Attend bi-annual meetings with clients to monitor services
- Develop and maintain a collaborative relationship with clients, serving as a role model and demonstrating positive, proactive, respectful communication
- Assess client needs and ensure continuity of care
- Maintain regular contact with collaborative partners
- Make referrals and authorize services based on client needs and personal outcomes
- Maintain records and document all services provided; consistently document all interventions, interactions and relevant information to the support plan in accordance with professional and organizational standards
- Must reside within the metro area
Essential for all Case Manager II positions:
- Valid driver’s license, excellent driving record and current auto insurance
- Pass all background and federal Human Services Office of Inspector General screenings to ensure no exclusion from working in federally funded health care programs
- Ability to meet all facility and position requirements
Education and Experience Requirements for Targeted Case Manager II position:
- Bachelor’s degree in behavioral science or related field
- 2,000 hours of supervised experience working with adults with severe and persistent mental illness
- OR
- Master’s degree in behavioral science or related field
Preferred Qualifications for Targeted Case Manager II position:
- Experience providing direct services to adults with serious and persistent mental illness
- Experience with case management and commitment proceedings
- Knowledge of Recovery, Motivational Interviewing and person-centered approach
Education and Experience Requirements for Hennepin, Dakota County & Saint Louis County Waiver Case Manager II positions:
- Bachelor’s degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field
- OR one year of experience as a social worker/case manager/care coordinator in a public or private social service agency
Education and Experience Requirements for Ramsey County Waiver Case Manager II positions:
- Bachelor’s degree in social work, special education, psychology, nursing, human services or related fields, plus one year of experience in the education or treatment of persons with developmental disabilities or related conditions
- Up to 112 hours of paid time off (PTO)
- Up to 48 hours of earned sick and safe time (ESST) in accordance with state and local requirements
- Up to 8 paid holidays per calendar year (4 company-designated holidays and 4 floating holidays prorated based on date of hire)
