Position Summary

Accurate Home Care is hiring an RN Admission/ Visit Nurse to provide 1:1 care, manage schedules, and serve patients within a 60‑mile radius of the Otsego office.

• Provide appropriate care through admission assessments, care planning, goal setting, evaluation, coordination of care, and emergency/safety plans.
• Complete OASIS assessments at prescribed time points, including admission/start of care.
• Identify potential referrals for services from other disciplines that have not yet been ordered.
• Collaborate with clients, families, physicians, and other members of the patient care team.
• Assess and monitor patients, maintain current documentation in the clinical record, and report concerns to the Director of Home Health.
• Provide instruction to patients and families on illnesses, treatment regimens, and proper health maintenance.
• Administer medications and treatments as prescribed by the physician within scope of practice and company policy.

• Licensed as a Registered Nurse (RN) in Minnesota.
• Basic Life Support (BLS) CPR certification for healthcare providers, renewed at recommended frequencies.
• Ability to lift, turn, position, and transfer up to 50 lbs.
• Fluent in the English language.
• Valid U.S. driver’s license with minimal moving violations and reliable personal vehicle.

• $45–$156 per visit.
• Mileage reimbursement with potential for company vehicle.
• Monday–Friday schedule with weekend rotation.
