Access Coordinator I

Position

Purpose:

The Access Coordinator ensures seamless progression for individuals entering services by guiding them through the intake process, supporting initial engagement, and rapidly addressing barriers to access. This role is focused on facilitating timely and efficient connections to appropriate services, acting as a bridge between initial contact and long-term care coordination. This position is designated as a hybrid role, combining remote and in-person work.

Specific onsite expectations are subject to ongoing evaluation and may evolve over time.

Monday - Friday 8:00 - 5:00 PM

3000 Ames Crossing Rd, Ste 6000, Eagan MN 55121

Must have physical presence in the Twin Cities area

Hiring Range: $24.20 - $26.14/ Hourly, based on qualifications, plus $1,000 hiring bonus to external candidates

Organization Relationships:
This position reports directly to a Program Supervisor or Manager and will interact on a regular basis with other members of an interdisciplinary team. This position works cooperatively with all program staff and other People Incorporated staff.

Key Responsibility Areas

Intake, Assessment and Early Engagement – 40%

  • Serve as the primary point of contact for individuals entering services, establishing early rapport and trust.
  • Guide individuals through the intake process, including completion of pre-screeners and documentation of basic health information.
  • Identify and address immediate barriers to service access (e.g., transportation, system navigation, insurance).
  • Ensure rapid access to comprehensive evaluation and connection to appropriate services.
  • Emphasize efficiency and short-term support during the early stages of engagement.
  • Follow up with individuals before and after comprehensive evaluation to support engagement and retention.

Service Coordination and Transition Support – 40%

  • Manage and track internal and external referrals from the evaluation process.
  • Facilitate and coordinate connections to services, including hand-off to long-term care coordination teams.
  • Maintain communication with individuals and service providers to ensure successful initiation of services.
  • Continue following client and providing appropriate coordination until there is a successful transition.
  • Complete any initial paperwork, assessments, and any other relevant documentation during the transition process.
  • Provide various skill-building, teaching, and/or coaching, as it relates to the individual’s goal and/or treatment plan.

Collaboration and Interdisciplinary Partnership – 10%

  • Work closely with interdisciplinary teams, program staff, and external partners to resolve barriers and enhance access.
  • Develop effective working relationships with internal and external referral resources.
  • Maintain regular contact with collaborative partners, including schools and community agencies.
  • Actively seek consultation from team members for complex cases.
  • Maintain communication with program management on advanced or complex cases.
  • Attend scheduled in-person team meetings and may be required to report onsite based on program needs, as determined by their supervisor.
  • Participate in trainings, supervision, and other required activities.
  • Maintain flexibility to adapt to changing onsite requirements.
  • Participating in weekend on-call coverage schedule as determined by your supervisor and program need.
  • Attendance is an essential function of the position.

Documentation and Compliance – 10%

  • Document all interactions, interventions, and relevant information in accordance with organizational and professional standards.
  • Maintain accurate and timely case notes in the Electronic Health Record.
  • Demonstrate the ability to meet established performance metrics and productivity standards.
  • Abide by HIPAA guidelines and organizational policies regarding client privacy and documentation.
  • Other duties as assigned.
