Minnesota Mental Health Clinics is excited to welcome a new Case Manager to our Horizons Targeted Case Management Program—a role where your work directly impacts the day-to-day lives of our community members navigating severe mental health challenges.

Case management is meaningful work, and it’s also demanding work. You’ll be working with clients facing complex systems, housing instability, financial barriers, while managing serious mental illness. This role requires someone who is self-aware, confident and resilient, as you help clients navigate the healthcare system, access resources, and be a reliable and dependable pillar in their journey.

As a Case Manager, you’ll do more than coordinate care—you’ll be a consistent advocate, problem-solver, and support system for your clients. You’ll help individuals access critical resources, build stability, and work toward meaningful, realistic goals that support independent living and quality of life.

Our clients are referred through Hennepin and Dakota County Social Services, and many are navigating complex systems. Your ability to guide, support, and empower them can dramatically change someone’s life.

Specific responsibilities include:

• Connect clients with available Hennepin and Dakota County resources to support their daily lives
• Provide ongoing assistance with mental health support, education, community and health programs, financial security, county and state insurance, housing, medication checks and refills, food security, insurance paperwork/renewals, etc.
• Provide Person-Centered Services by supporting clients in accessing available resources
• Communicate with clients and other providers via email, phone, text, and video chat
• Conduct check-ins with clients both in person and via phone/video
• Complete paperwork and documentation with a focus on quality and compliance in a timely manner, including required assessments every 6 months
• Attend weekly staff meetings and supervision; collaborate with team members and supervisors to share knowledge and problem-solve
• Learn about available resources for clients and how to access them; demonstrate resourcefulness in identifying solutions
• Develop individualized treatment and care plans based on client needs
• Coordinate client care with other programs, resource agents, and supports (e.g., family, school, caregivers, doctors, therapists, etc.)
• Support with projects that help improve the broader TCM program (e.g., build positive culture among the team, improve efficiency in TCM workflows, test and adopt new technology or tools, partner with community resources)
• Hybrid home and in-office setting with travel to meetings, conferences, and client locations including homes, hospitals, clinics, and other community sites
• This position requires regular driving to meet with clients, often multiple times a week, including occasional transportation of clients
