Join Wasatch Behavioral Health as a Part-Time Case Manager at our Eagle Mountain Family Clinic in Eagle Mountain, Utah! With a competitive wage of $21.36–$29.88/hour
, this role is a great opportunity to make a meaningful difference in the lives of individuals, children, and families navigating mental and behavioral health challenges. The Eagle Mountain Family Clinic provides a variety of therapeutic services for clients affected by trauma and/or mental illness. As a Case Manager, you'll help clients access community resources, coordinate services, and connect with the support they need while working alongside a collaborative multidisciplinary team to help clients build stability, confidence, and progress toward their personal goals.

Schedule: Monday-Friday, Hours are Varied
Compensation: $21.36–$29.88 DOE and Licensure

What You'll Do as a Case Manager
  • Conduct client assessments and help create individualized care plans.
  • Coordinate services and collaborate with other providers to support client needs.
  • Offer ongoing support, guidance, and follow-through with clients.
  • Connect clients and families with community resources and supportive services.
  • Work closely with a supportive, multidisciplinary team.
  • Help clients navigate appointments, services, and other resources.
  • Build positive relationships with clients and families through compassionate, client-centered support.
  • Help clients build stability, confidence, and progress toward personal goals.
  • Maintain accurate documentation, case notes, and client records.
Qualifications

Applicants must meet ONE of the following:

Note:

Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.

  • Bachelor's degree with Social Service Worker (SSW) license
  • Bachelor's degree in any field with willingness to pursue SSW licensure
Additional Requirements:
  • Valid driver's license
  • Basic understanding of Social Work principles
  • Familiarity with social welfare policy and human development
  • Ability to problem-solve and maintain a client-centered approach
  • Spanish speakers may qualify for increased wages after testing

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen, motor vehicle record review and criminal background check. After hire you must complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing. Upon receipt of a conditional offer of employment in the position, you must provide an official copy of licensure or certification.
